What’s required for the physical?
-Valid drivers license
-Drivers must speak and write in English
-CPAP users must bring a printout of their previous 12 month CPAP compliance report
-Be able to fill out a recent health history
-Glasses/contacts if worn while driving
– Urine sample (to be collected at office), which is not a drug test
-List of current medications: prescription and over the counter, including supplements
-The cost of the DOT/CDL physical is $125
